There is only one Decompression Chamber in Bali at Sanglah hospital which is far from Tulamben. Via the Bali Scuba bus it should take approximately 2 hours. So delegate someone to prepare the bus as soon as possible to reduce lost time. 

BIMC Hospital is a well known, private hospital in Kuta. It is a long drive, however they have well equipped ambulances which operate 24 hours a day

Bali Scuba is Five Star PADI Dive Center that we will be diving through. They are highly reccomended and are very reliable when it comes to accident management. It would be recommended to report the incident to them as soon as possible

DAN is available 24-hours a day, 7 days a week to assist in the event of an accident. Providing Worldwide Emergency Evacuation Coverage, Dive Injury Insurance Services, 24-hour diving emergency hotlines and non-emergency diving medical advice for all members.

Follow the diving incident flowchart below.

Locate the patients identification cards, personal belongings / Anything relevant will help. ie: dive history. 

Record any other information on the information sheet below, if patient is can't, ask around for someone that knows the diver.

Locate and Secure the patient gear - Do not disassemble it.

Cooperate with the authority. Do not give personal judgement, only what really happened.

Write an accident report to PADI statement facts that you observe and not your own judgement.

Give authority or evacuation personnel the patient information.
